I usually apply concealer first, then the foundation to blend in over/with the concealer.

It depends on the person. I like to apply my concealer first and then my foundation, the technique is called pre-concealing. I then put on foundation and blend so that the concealed parts aren’t obviously a different color from the rest of my face.
I do concealer first, but a trick I’ve learned recently is to set it first with translucent powder. I used to have the problem of being in a hurry and not letting my concealer set long enough after I blended it, so when I would apply foundation I would smudge it and move it out of the way. The translucent powder helps it to stay in place. I say this because I know alot of girl who do concealer AFTER their foundation because of this very problem.
Most days, I will just wear concealer – sometimes with a tinted moisturizer – I don’t have gorgeous skin – just don’t like foundation too much. When I do wear foundation, the concealer always goes on first. Like Ashley Samantha, I have found that I have to let my non-tinted moisturizer set first or my concealer will separate.
I used to do foundation first, but realized that the concealer was such a noticeably different color that it had this weird reverse-raccoon effect under my eyes. Now, I do concealer first, let it dry, and then apply foundation to blend. Or on days when I’m feeling particularly fresh-faced, I do concealer, very lightly!, and then put some powder on top to blend.
i like using foundation first then concealer, this way i am only “concealing” what is necessary & not using too much concealer when only sheer touch of foundation is needed.

I do concealer first, then a tinted moisturizer. Three hours later the product has separated a little under my eyes. I still have not found the best concealer for me. I have gone from $65.00 concealers to less expensive, and none work well. They are either too light or too dark. I have light/medium olive skin.
